Certified Home Health Aide
You will participate and work with a team to deliver hands-on, compassionate, end of life care while supporting patients, family members, and other community caregivers as they navigate the complexities of their journey.

Position Qualifications:
Meets California Department of Public Health (CDPH) requirements for Home Health Aides in a Medicare-certified Home Health Agency.
Has successfully completed a CDPH-approved Home Health Aide (HHA) training program consisting of a minimum of 75 hours (including at least 16 hours of supervised practical training), and has passed the required competency evaluation, in accordance with 42 CFR §484.80 and California regulations.
Currently certified as a Home Health Aide (HHA) by the California Department of Public Health (CDPH) and listed on the CDPH Home Health Aide Registry (required).
Ability to read, write, and follow written instructions, and to accurately document care provided in compliance with Medicare Conditions of Participation.
Demonstrates understanding of hospice philosophy and is comfortable providing specialized care to terminally ill patients (if applicable to role).
Provides satisfactory employment references.
Is self-directing and able to work with minimal direct supervision while maintaining compliance with agency policies and patient care plans.
Demonstrates emotional maturity and comfort with issues of death and dying, maintaining a calm and professional manner in patients’ homes.
Shows empathy and compassion for the needs of ill, injured, frail, and impaired individuals.
Maintains current CPR certification in accordance with agency and Medicare requirements.
Demonstrates tact, patience, professionalism, and good personal hygiene.
Holds a valid California driver’s license and has access to a reliable, insured vehicle that meets organizational requirements and is in good working order.
