Responsibilities
- Plan and conduct segments of treatment programs to restore and improve physical, social, and mental functions for patients.
- Develop and implement OT treatment plans aligned with patient goals and department objectives.
- Serve as a co-leader in an interdisciplinary approach to patient groups, collaborating with therapy colleagues, nursing, and other care team members.
- Assess patient needs, document progress, and adjust interventions to maximize functional outcomes and independence.
- Participate in discharge planning and goal-setting discussions to support smooth transitions of care.
- Maintain accurate, timely records and communication to ensure continuity of care across shifts and teams.

Requirements
- Active Occupational Therapist license in California.
- Ability to practice in a SNF/interdisciplinary care setting.
- Experience or demonstrated comfort in collaborating within a multidisciplinary team.
- Strong assessment, treatment planning, and documentation skills for long-term care residents.
- Availability for a 40-hour workweek during the assignment period.
- Effective communication and patient-centered approach to therapy.
