Tribal Adult Resources Nurse (LVN)
JOB DESCRIPTION Title: Tribal Adult Resources Nurse (LVN) Department/Division: Administration Status: Full-Time Salary: Hourly / DOE Supervisor: Chairman POSITION SUMMARY: Applicants must have an active California LVN license and be in good standing. The Tribal Adult Nurse will work as part of the tribal members overall health care team and is responsible for performing routine technical nursing duties and assisting with other related duties involved in providing patient care for at-risk Tribal adult dependents. Duties may include responding to reports of suspected elder and dependent adult abuse, including neglect, self-neglect, and financial abuse. ESSENTIAL DUTIES AND RESPONSIBILITIES: Maintains a variety of confidential information (e.g., medical information, completed standard forms, records of communication, contact information, certificates, determinations, referrals, and requests made, caseload logs, photographs, or any other service-related information/documents, etc.) by organizing hardcopy data using a Departmental case management format and electronic data utilizing computerized systems in order to document information and ensure the security of the documents in accordance with Tribal regulations and Department policies and procedures. Offers client(s) various services provided by Tribal Adult Services and/or community resources (e.g., LVN level nursing services, coordination of care, and immediate and ongoing assessments and referrals, etc.) throughout the life of the referral/case in order to address the clients or familys immediate or long-term basic needs and monitors compliance with case plan activities by helping clients identify their needs for other services and by either providing the client pertinent information to obtain the assistance or coordinating with other departments or agencies/community resources to arrange for service delivery. General responsibilities may include feeding patients, assisting with bathing and grooming, giving injections and medication, caring for wounds, recording vital signs, tracking medical history, monitoring health status, and keeping detailed documents on patient progress. Living assistance responsibilities may include ordering and delivering incontinence supplies, ordering or shopping for groceries, picking up and delivering prescription medication, escorting patients to doctor appointments, and ordering the proper medical gear for patient quality of life and care. Educate Elders on health and wellness issues. Provide Adult centered education to, advocate for individuals living in the community to the full extent of your training, plan, organize, develop, facilitate, and direct activities that are of interest to the individuals in order to provide meaningful ways to spend time as recreation, exercise, relationship-building, and enjoyment. Services are provided in accordance with the individuals assessment, personal preferences, and care plan goals. Manage multiple Tribal Elders and Adults with needs simultaneously, requiring multiple home visits. Communicate with the family to provide resources and support, as well as relay information to doctors and other healthcare professionals who are part of the treatment plan. Possibility of an occasional need to act as interim conservator over Tribal Members as assigned by the Tribal Court. Perform other duties as assigned by the Chairman. KNOWLEDGE, SKILLS, CERTIFICATIONS AND ABILITIES: Knowledge of and the ability to perform nursing procedures for which the vocational nursing course has provided the necessary degree of skill and judgment; performance shall be limited to those acts for which he or she has been prepared. Knowledge of accepted procedures for disposing of used supplies. Ability to function independently and have flexibility, personal integrity, and the ability to work effectively with residents and support agencies. Ability to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people. Ability to make independent decisions when circumstances warrant such action. Ability to record observations completely, accurately, legibly, and concisely. Ability to communicate effectively in person and over the telephone. Ability to demonstrate quality-evidenced-based nursing care to adults. Ability to initiate emergency protocols in response to patient situations. Ability to perform duties outlined in the care plan with expertise and diligence. Ability to prepare, administer, and record medications per MD order and agency protocols. Adheres to agency standards of care, policies, and procedures. PHYSICAL ABILITIES: Prolonged periods of sitting at a desk and working on a computer. May require prolonged periods of standing, moving, and bending. Must be able to lift up to 75 pounds at times. MINIMUM QUALIFICATIONS: Current California unrestricted license as a Licensed Vocational Nurse or Registered Nurse in good standing. Prefer two (2) years of supervised clinical experience, preferably with elders and adult dependents. A minimum of one year of experience in an acute care facility. Current CPR certification. Must have the physical ability to execute the full performance of the positions duties. Must be able to pass a background check. Must be able to pass a Livescans background check, if requested. Must possess a valid California Class C drivers license in good standing. Must acknowledge and agree to maintain a Drug-Free workplace (including Marijuana) as a condition of employment with the Pala Band of Mission Indians. Must pass pre-employment and random drug testing during employment. Must be culturally competent with high ethical behavior. Indian preference will apply. The Pala Band of Mission Indians will give hiring preference to fully qualified applicants that are Pala Tribal Members. OTHER REQUIREMENTS: All employees are expected to follow the Tribal Personnel Policies and Procedures of the Pala Band of Mission Indians and must adhere to any additional applicable addendums.
