Care Coordinator - Engagement Specialist
Hours : 40 hours per week. Hours are Monday, Tuesday, Thursday, and Friday, 8:30 am to 5:00 pm; and Wednesday, 10:30 am to 7:00 pm.
Pay Rate : $28.85 to $29.13 per hour
Education: Bachelor’s degree in a mental health-related field such as Psychology, Social Work, or Counseling required

As a Care Coordinator - Engagement Specialist, you will:
- Contact clients who have missed appointments or are disengaged in services through phone calls, letters, or other approved communication methods.
- Identify reasons for disengagement (e.g., transportation, financial, scheduling) and collaborate with clients and the treatment team to address these challenges.
- Work closely with the administrative and clinical staff to schedule appointments, update treatment plans, and ensure continuity of care.
- Provide face-to-face and field-based services for the purpose of re-engaging clients, as needed.
- Document all services and outreach efforts using concurrent and collaborative documentation; maintain all documentation and bill for services according to established policies and procedures in a timely manner.
Additional Qualifications and Skills
- At least one year of related experience and/or training
- Must be able to work in a fast-paced and collaborative environment
- First Aid/CPR certification strongly preferred; required post hire. Certification must be kept current throughout employment.
- Bilingual Spanish preferred, but not required
- A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position
- Proficient computer skills, including MS Office
- Able to pass criminal/background clearance

Our Adult Outpatient Programs (Areta Crowell Wellness Recovery Center, Douglas Young Wellness Recovery Center, Heartland Wellness Recovery Center, Jane Westin Center, Maria Sardinas Wellness Recovery Center, and South Bay Guidance Center ) provide behavioral health services including psychiatric and clinical assessment, treatment planning, ongoing medication services, individual and group counseling, case management, crisis intervention, mobile outreach, vocational support, and peer support. We deliver fully integrated services to adult clients diagnosed with a serious mental illness, as well as individuals with co-occurring, mental health, and substance use disorders.
All of our outpatient programs also provide full-time walk-in services. Our programs utilize evidence-based practices to assist our clients in reaching their goals and to live the highest quality of life while managing their mental illness.
