About the Position
The RN Case Manager coordinates and manages patient care, develops care plans, monitors treatment progress, and collaborates with healthcare teams to ensure safe, efficient, and high-quality outcomes. The role includes patient education, discharge planning, and working with insurance providers to optimize care.
Requirements
Active RN license
2–4 years acute care experience
BLS required; CCM certification preferred
Strong care coordination, communication, and critical-thinking skills
