Hospice Community Liaison
- Establishing strong relationships with physicians, hospitals, nursing facilities, and other referral sources.
- Seeking and securing new referrals by educating potential referral sources about hospice services and benefits.
- Implementing marketing strategies, attending trade shows, and participating in community events to raise awareness of hospice services.
- Informing the community about hospice/home health care, services, and benefits.
- Tracking and reporting referral and admission data to identify trends and growth opportunities.
- Working with the clinical team to ensure a smooth transition to hospice/home health care.
- Strong communication and interpersonal skills.
- Excellent presentation and sales skills.
- Ability to build and maintain relationships.
- Understanding of the hospice care continuum and related regulations.
- Experience in healthcare marketing or sales is beneficial.
- Associate's degree or equivalent experience.
