About the Position

TravelNurseSource is working with Lancesoft Inc to find a qualified Home Health RN in Torrance, California, 90501!

Torrance Memorial Medical Center is seeking an experienced Home Health Travel RN for a 13-week assignment. Unlike traditional "pay-per-visit" roles, this position follows an  hourly model , requiring travelers to be on-site/working from 08:00 to 16:30. This unit manages a high volume of lower leg wound care, IV therapy, and chronic disease management (Heart Failure/Diabetes).

Schedule & Shift Details

• Shift: Days (5x8-Hour) | 08:00 – 16:30
• Duration: 13 Weeks
• Start Date: 05/25/2026
• Weekend Requirement: Every other weekend (2 weekend shifts per contract)
• Caseload: 5–6 patients per day

Key Responsibilities

• Clinical Care: Perform high-volume wound care (including Wound V.A.C. management), PICC line dressing changes, and IV therapy.
• Assessments: Conduct Start of Care (SOC) assessments (expect 3 starts in a 12-hour equivalent or high-efficiency completion), Resumption of Care, and Routine visits.
• Management: Oversee anticoagulation therapy, phlebotomy, and glucose management.
• Documentation: Complete all visits and charting within the 8-hour scheduled shift using OASIS-E1 guidelines.

Compensation Notes

• Hourly Integrity: Travelers must start at 08:00 sharp and work the full 8-hour shift. Charting from home is typically not expected; visits should be completed within the shift.

Requirements

Experience & Education

• Minimum Experience: 2 years of Home Health RN experience with high-volume caseloads (No exceptions).
• Travel Experience: Minimum 1 year of Home Health specific travel experience required (General travel experience will not be accepted).
• Licensure: Active California RN License required at time of submission.
• OASIS Proficiency: Recent OASIS experience is required. Candidates must pass an OASIS Exam to be eligible for an interview (Focus: Oasis E11, G-Section, GG Section, and M18).

Certifications & Skills

• BLS: American Heart Association (Required).
• Technical Skills: Proficiency in Wound V.A.C., G-tube care, urinary catheter management, and IV/Central line management.
• Physical Requirements: Ability to stand for long periods, squat, and climb stairs without assistance.

Travel & Territory Details

• Territory: LA County (Coverage radius of 5–20 miles).
• Driver’s License: Must have a current, valid driver's license and reliable transportation.
