APN - Home Based Primary Care

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) in Home Based Primary Care (HBPC) practices in Extended Care and Palliative Medicine Service to provide a full range of health care services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
• The NP provides advanced clinical practice - consultation - and management
• The major duties and responsibilities include - but are not limited to: Independently manages a panel of patients with acute and chronic medical - social - and behavioral conditions - with an emphasis on caring for older - frail - homebound patients
• Visits patients in their homes in the community
• Assesses - identifies - and diagnoses patients' general medical problems and defines the medical management
• Develops and implements an individualized treatment plan to achieve optimal patient health outcomes
• Demonstrates leadership and works collaboratively with HBPC interdisciplinary team members
• All other duties as assigned
