Your Role as a Home Health Occupational Therapist

As an OT in our home health team, you'll work closely with Case Managers to assess and treat patients in their homes. Your responsibilities will include:

Evaluating and addressing patient needs to ensure a safe and productive home environment.

Assisting with home safety, medication management, and pain control.

Providing training for family caregivers on dementia care and chronic disease management.

Collaborating as part of an interdisciplinary care team.

Acting as a patient advocate and educator to enhance overall well-being.

What We're Looking For

Current, unrestricted CA Occupational Therapist License

BLS/CPR Certification

Valid CA Driver's License

Knowledge of local community resources

Preferred Experience:

Home health care or high-acuity patient care

Working with an interdisciplinary team

Case management fundamentals

Essential Skills:

Self-motivated and organized

Strong critical thinking and leadership abilities

Empathetic with excellent interpersonal skills
