Home Care Coordinator
The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, and promoting autonomy, quality of life and the ability of individuals to live in their communities.
Hourly Pay Range: $23.40 - $35.10
The Position : Under the direction of the Director of Home Care, the Home Care Coordinator (HCC) conducts home safety evaluations in the participant’s place of residence to determine home care and DME needs. The HCC is an integral member of the Interdisciplinary team and contributes to participants plan of care to ensure that participants are receiving care that is appropriate for their safety and maximum independence in their homes.
Duties and Responsibilities:
- Participates as a member of the Interdisciplinary Team (IDT), responsible for care evaluation, planning, management and communication of in-home care needs. Includes ongoing evaluation of home environment in relation to the changing needs of the participant.
- Completes the initial, annual and semiannual screenings and evaluations of participants timely and as assigned.
- Assesses changes in participant’s level of care and home care needs based on results of the home care assessment and service plan tool and presents to the IDT for discussion and approval of any proposed change in the Plan of Care.
- Evaluates participants’ physical and mental health within their scope of practice.
- Collaborates with Personal Care Attendants on delivering the service plan of participants and reports any concerns in a timely manner.
- Attends team meetings, IDT meetings, Home Care meetings, family conferences, and other meetings as required by the Center.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Documents all participant and family contact accurately and on time in the participant medical record.
- Participates in the Quality Improvement Program as assigned.
- Maintains the confidentiality of all company procedures, results, and information about participants or families.
- Responsible for assuring Durable Medical Equipment (DME) for participant homes are effectuated timely.
- Complies with all agency training requirements.
- Maintains safe working environment by following CEI’s safety P&Ps.
- Broad knowledge base of activities of daily living (ADL/IADL) of the frail elderly. Knowledge of durable medical equipment. Able to work independently with minimal supervision. Skilled in identifying problems and recommending solutions.
- Skilled in establishing and maintaining effective work relationships with participants, interdisciplinary team members, and family caregivers
Q ualifications :
- Graduate of an accredited school of professional nursing and possess a current California Licensed Vocational Nurse License Preferred. Comparable experience and certification will be considered.
- Valid California driver’s license, motor vehicle insurance and reliable transportation required.
- Minimum of one year experience in a home care agency, primary care clinic, or health care facility preferred.
- Current CPR certification is required within 6 months of employment.
- Maintains eligibility for licensure by obtaining the required Continuing Educational units as needed.
