Home Health RN | Travel Contract

Position Overview

Experienced Home Health RN needed to provide skilled nursing care to a primarily geriatric patient population. Candidates must have recent OASIS and Homecare Homebase (HCHB) experience.

Assignment Details

• Specialty: Home Health
• Experience Required: Minimum 1 year of recent Home Health experience
• First-Time Travelers Accepted: No
• Guaranteed Hours: 40 hours per week
• Productivity Expectation: Minimum 30 points per week
• Average Patient Visits: 5–7 patients per day (please confirm during interview)
• On-Call Requirement: Yes (details discussed during interview)
• Territory: Up to a 60-mile radius from the assigned office/hub
• Local Candidates: Accepted at the travel rate
• License: Active state RN license required at the time of submission

Required Experience

• Recent OASIS experience – Required
• Homecare Homebase (HCHB) EMR experience – Required

Patient Population

• Primarily Geriatric patients

Additional Requirements

• Must be flexible with assigned territory
• All documentation/charting must be completed by 9:00 AM the following day
• Timely communication with the clinical team is required
