Responsibilities:

• Perform comprehensive patient assessments and care planning
• Collaborate with interdisciplinary team (IDT)
• Coordinate care across acute, post-acute, and long-term settings
• Conduct occasional home visits
• Ensure high-quality, patient-centered care

Qualifications:

• MD or DO – Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics
• Board Certified or Board Eligible
• Active California license (or ability to obtain)
• Eligible to provide care to Medicare/Medi-Cal populations
• Strong interest in value-based and geriatric care
