Responsibilities
- Provide comprehensive nutritional care to patients identified with nutritional risk or special needs
- Conduct initial and ongoing nutrition assessments and reassessments; develop, implement, and document the nutrition care plan
- Educate patients and families on nutrition care, disease management, and healthy lifestyle choices
- Communicate and collaborate with the interdisciplinary care team to ensure integrated patient care
- Document all interventions in accordance with policy and accepted practice standards
- Deliver clinical care in a skilled, effective manner using appropriate clinical judgment and technical skills

Requirements
- Active Registered Dietitian license (California)
- Clinical experience in nutrition assessment, care planning, and documentation
- Ability to work in a home healthcare/hospice setting and adapt to patient- and family-centered care
- Strong communication and education skills for patients, families, and the care team
- Availability to work 8-hour days (40 hours per week) on a travel assignment
- Willingness to travel to assigned area and participate in a 13-week contract
