HT Case Manager
Covenant House California (CHC) is a nonprofit organization that provides sanctuary and support for youth overcoming homelessness, ages 18-24. As this position provides direct care to young people that are 18-24 years old, we require that applicants are at least 25 years old.

POSITION SUMMARY:
The Human Trafficking Case Manager is responsible for working with youth experiencing homelessness and trafficking through our street outreach and shelter housing program. CHC strives to meet the needs of youth that are experiencing homelessness and trafficking by providing supports around housing, medical, mental health, employment and educational goals and overall wellbeing. Our work with youth that are trafficking survivors is very focused on being a first contact or experience/respite from the life, being willing to continually offer support, care, unconditional love in ways that support their own journey through stages of change and harm reduction. Essential functions of the position include intake and assessment, case planning, documentation with data entry, and basic individual and group counseling.
ESSENTIAL FUNCTIONS:
Provide case management services.
The goal at Covenant House California is to develop positive relationships with the residents through the effective utilization of the case management system. This system includes:
a. Intake:
Welcome youth to the program;
Review intake documentation
b. Full Assessment:
Review and complete the Intake/Assessment and the accessing of resources for the youth.
c. Individual Service Plan:
Develop an Individual Service Plan (ISP) with the youth upon completion of assessment;
Revise and update the ISP as necessary. An updated plan is written, at minimum, after two weeks of intake, and upon the transition to a new Phase in the program;
Maintain a calendar of the youth’s work schedule, appointments, etc., in conjunction with the case plan;
Develop and maintain knowledge of and connection to community resources;
Make appropriate referrals for services within CHC and to community agencies;
Engage in positive transition planning with the youth.
d. Transition Plan:
Directly help the identified youth by: locating housing, filling out housing applications, complete housing habitability standards;
Providing mediation with landlords and neighbors;
Providing written information about landlord/tenant rights and responsibilities;
Reviewing and understanding the requirements of the lease;
Obtaining, interpreting and correcting as needed, rental and credit history;
Developing a household budget and reducing expenses to the extent possible; reducing or re-negotiating debt and/or obtaining other consumer credit counseling assistance;
Providing information and referral to employment and free or reduced-cost goods and services.
Partner with community and partnerships to negotiate and support payment of security deposits, utility assistance, rental assistance, furniture needs, and other housing costs as appropriate to the situation.
e. Documentation:
Monitor and document regular progress of youth’s case plan and maintain case management files for all youth (HMIS and ETO) assigned to his/her case load.
Monitor program outputs and ensure that program outcomes are met and that there is complete documentation specific to external grants as required.
Document pertinent information in Critical Incident Reports, Follow-Up forms, etc., as necessary.
e. Case Review:
Prepare and present assigned cases at case reviews.
f. Group Facilitator:
Facilitate or support group activities with youth on a regular basis.
Be on time and present for assigned shifts as well as all scheduled meetings, including off-site community collaborative meetings, staff meetings, and training.
Perform duties as assigned to meet the needs of youth and ensure adherence to the program expectations. This includes, but is not limited to:
facilitating group case management meeting with youth.
ensuring the cleanliness and safety of the apartment.
providing crisis intervention, as necessary.
Other program duties as assigned by the Director of Case Management Services
JOB REQUIREMENTS:
EDUCATION : BA degree in Psychology, Sociology, Social Work or related field preferred.
EXPERIENCE : Minimum of 3 years of case management or relevant experience, preferably with homeless or runaway youth. Must be a self-starter and team player with the ability to work with other staff. Leadership skills and program development experience are preferred.
SKILLS : Knowledge of adolescent development, community resources, and case management skills. Must have good organizational skills and the ability to communicate with members, clients, vendors and the public. Communication with and about youth must reflect a “strength based” and “trauma informed” approach. Bilingual English/Spanish preferred.
EXPECTATIONS:
· Conduct regular home visits by driving to meet youth at their residences as required to provide case management support and ensure ongoing engagement.
· Participate in weekly one-on-one check-ins and case management meetings with your immediate supervisor to receive guidance, support, and accountability.
· Adhere to all Covenant House California (CHC) policies and procedures in accordance with the Interim Housing Scope of Required Services.
· Attend and complete all required trainings assigned by the Los Angeles Homeless Services Authority (LAHSA), the Centralized Training Academy (CTA), and all mandatory CHC trainings.
Case Management Performance Expectations:
· Intake/Assessment: Complete within 3–7 days of a participant’s intake.
· Success Plan (Housing Plan): Develop within 30 days of intake and update every 30 days thereafter in both HMIS and ETO systems.
· Wellness Survey: Complete every 30 days.
· Case Notes: Document case notes in the system at least once per week to reflect services provided and participant progress.
