Key Responsibilities
- Conduct initial assessments and develop personalized care plans for patients.
- Coordinate and monitor the implementation of care plans to ensure effective and efficient delivery of services.
- Collaborate with healthcare providers, patients, and families to facilitate optimal healthcare experiences.
- Manage case documentation and maintain accurate records of patient interactions and care interventions.
- Provide education and support to patients and families to enhance understanding of conditions and care plans.
- Evaluate patient progress and amend care plans as necessary based on their outcomes and advancements.
- Advocate for patientsâ needs and preferences throughout the care process.
- Stay updated with current nursing practices and state regulations affecting case management.
Essential Qualifications
- Bachelor's Degree in Nursing
- Certification as a Case Manager is required
Desired Experience
A minimum of 2 years of experience in a nursing role with a focus on case management is essential.
