Position Overview

Under the direction of the Director of Nursing, the LPN Nurse / Medical Records LPN is responsible for preparing, maintaining, and closing medical records, while ensuring all documentation is accurate, complete, and compliant.

Key Responsibilities

1. Prepare medical records upon admission and ensure all necessary forms are present
2. Enter resident information into the computer system for new admissions
3. Review and verify records, MARs, and TARs for accurate transcription and order entry
4. File, organize, and retrieve records as needed; reduce records monthly
5. Compile discharge records, close files, and ensure proper documentation is complete
6. Perform record audits for accuracy, completeness, and timely signatures
7. Ensure physicians complete and sign discharge summaries

Qualifications

1. Licensed Practical Nurse (LPN) required; long-term care experience preferred
2. Strong knowledge of medical terminology and physician orders
3. Excellent telephone, communication, and interpersonal skills
4. Strong organizational and filing skills
5. Basic computer proficiency required for electronic documentation and data entry
6. Professionalism, patience, and a positive attitude toward elderly and disabled residents
7. Must be able to read, write, speak, and understand English
8. Must be at least 18 years of age
9. Must be able to meet the Essential Job Functions and Physical/Sensory Requirements of the role
