Requirements

• Year of experience: 2 Years
• 2-year acute care Hospital Case Manager experience performing Care Coordination and Discharge planning
• Travel Experience Required
• HOSPITAL EXPERIENCE IS A MUST
• Current California Registered Nurse License
• Certfications: BLS
• Charting System: Epic
