Physical Therapist (PT) - Home Health

Assignment Duration

This is a 13-week contract position.

Schedule

Monday to Friday, 5 x 8-hour shifts, guaranteed 40 hours per week.

Job Description

The Physical Therapist will provide comprehensive home health services in a specified coverage area, performing evaluations, treatment plans, and developing therapy interventions for patients in their homes.

Responsibilities

• Conduct initial patient assessments and evaluations.
• Develop and implement individualized treatment plans.
• Document patient progress in Homecare Homebase (HCHB) according to guidelines.
• Ensure productivity levels meet the expected points per week.
• Participate in case conferences and other team meetings as required.
• Provide education and support to patients and families.
• Maintain compliance with all applicable regulations and policies.

Required Experience / Certifications / Licensure

• Valid Physical Therapist license in the state of practice.
• Home health and OASIS experience required.
• Experience with Homecare Homebase documentation is required; candidates may be considered without HCHB experience at a reduced rate if manager is willing to train for the first two weeks.
