What You'll Do: As a Physical Therapist Case Manager, Level 2, you'll partner with other clinicians to provide skilled services to individuals in their homes. You'll evaluate and address their needs, helping them to live safely and productively. Your expertise will be essential in: Performing in-home evaluations and developing comprehensive care plans.
Working with Physical Therapy Assistants (PTAs) to implement care plans effectively.
Helping patients improve mobility, movement, and regain independence.
Creating detailed treatment and care plans tailored to each patient's needs.
Communicating and coordinating with patients' physicians to ensure quality care.
Evaluating and reporting patient progress to MDs.
Supervising 2 Physical Therapy Assistants (PTAs) ensuring they are meeting performance standards.
What You'll Bring: Current, unrestricted CA Physical Therapist License.
1+ year of experience supervising PTAs in a home health environment (a plus).
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Understanding of individual development, human behavior, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience in home health care (highly preferred).
Experience with high-acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case management fundamentals (highly desired).
Self-direction, empathy, and critical thinking skills.
Outstanding interpersonal and leadership skills.
Strong organizational skills.
