Responsibilities
- Conduct comprehensive speech, language, voice, and cognitive-communication assessments for residents.
- Develop and implement individualized treatment plans aligned with resident goals and care plans.
- Provide direct individual and group therapy to residents and document progress in the patient record.
- Reassess residents regularly and adjust therapy frequency, goals, and discharge planning as appropriate.
- Collaborate with the interdisciplinary team to coordinate care, attend care conferences, and modify plans as needed.
- Ensure compliance with documentation standards, regulations, and facility policies.

Requirements
- California licensed Speech-Language Pathologist or eligible for licensure in California.
- Ability to work in a skilled nursing setting with older adult population.
- Willingness to participate in a travel assignment and meet scheduled hours.
- Strong communication, documentation, and collaboration skills.
- Proficiency with common therapy documentation systems and electronic records.
- Availability to work the stated shift (8:00 a.m.–4:30 p.m.).
