About the Position
TravelNurseSource is working with Compunnel Healthcare to find a qualified Case Manager RN in Santa Rosa, California, 95405!
Note: 2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment.
• Ability to organize and participate in family meetings.
• Participate in the development of the treatment plan.
• Experience with HMOs, IPAs, etc.
• Competent with arranging DME, SNF and ambulance transport. Must be able to perform this task independently.
