PHYSICAL THERAPIST
Ramona Rehabilitation and Post Acute Care Center is a 5-star skilled nursing facility dedicated to providing exceptional care and rehabilitation services to our residents. Our team is committed to excellence, compassion, and innovation in delivering personalized care. We are seeking a motivated and dedicated Physical Therapist to join our rehabilitation team and contribute to our mission of enhancing the quality of life for our residents.
WE ARE OFFERING A $6,000 SIGN ON BONUS. DETAILS DISCUSSED AT TIME OF INTERVIEW.
Key Responsibilities:
- Develop and implement individualized treatment plans to improve residents' mobility, strength, and overall physical function.
- Perform comprehensive evaluations and assessments to identify physical therapy needs and goals.
- Collaborate with interdisciplinary teams, including occupational therapists, speech therapists, nurses, and physicians, to ensure holistic care.
- Provide education and training to residents, families, and caregivers on therapy techniques and strategies for optimal recovery.
- Maintain accurate and timely documentation of therapy sessions, progress notes, and discharge plans in compliance with regulatory standards.
- Utilize evidence-based practices to deliver effective rehabilitation services.
- Foster a supportive and motivating environment that encourages residents to achieve their highest potential.
- Participate in team meetings, quality improvement initiatives, and ongoing professional development activities.
Qualifications:
- Licensed Physical Therapist (PT) in CA (or eligibility for licensure).
- Doctorate in Physical Therapy from an accredited program.
- Experience in skilled nursing, rehabilitation, or geriatric care preferred but not required.
- Strong interpersonal, communication, and teamwork skills.
- Commitment to providing compassionate, patient-centered care.
- Knowledge of relevant documentation and compliance standards, including Medicare regulations.
