Requirements

Years of Experience: Minimum 2 years Acute Care Case Management / Discharge Planning in a hospital setting

Note: 2 years Acute Care Case Management/Discharge Planning Charting System: Required , Epic Travel Experience Required: Yes Licensure: California RN license required
