Totally Kids Specialty Healthcare is a small pediatric sub-acute hospital with 45 patients. We are the ONLY long-term care hospital dedicated entirely to children. Our residents' ages range from newborn to 21 years of age and we are dedicated to enriching the health and quality of life for our kids. We are looking for a Social Worker to become a part of our facility's compassionate caregiving family. The Social Worker acts as a liaison and representative of the residents’ social interests and plans, organizes, develops, and directs social services in accordance with current federal, state, and local standards governing the facility, and as may be directed by the Administrator, to ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basis.

Requirements:

Education/Training: Bachelor of Social Work, BSW

Knowledge:

• Demonstrates ability to work with:
• Residents / Staff in a one-to-one and/or small group setting.
• Interdepartmental personnel toward team cohesiveness;
• Outside agencies; DCFS; schools; attorneys; Medi-Cal office

Physical Requirements:

• Must be able to crouch /stoop as required.
• Lifting Requirements: Must be capable of lifting or carrying objects that weigh up to 45 pounds.
• Must be able to discern colors.
• Physical skills necessary to operate all equipment: must be able to stand, reach in all directions, lift, and carry objects. Must have fine motor skills.
• Must have 20/20 vision (correction devices allowed) and be able to appropriately read patient information, posted materials, and other written information.
• Must hear (correctional devices allowed) and comprehend well enough to give and receive information for appropriate completion of duties.
• Must be able to speak, read, write, and listen in English with a skill level essential for communication in a sub-acute facility.
• Must have normal physical stamina.
• Must be able to bend and twist.
• Must have full use of both hands.
• Must be able to walk intermittently, six to ten hours per shift.
• Must be able to perform chest compressions and hand resuscitator ventilation during CPR.

Duties Summary:

• Plan, develop, organize, implement, and evaluate the social services programs and activities.
• Ensure ongoing quality assurance programs and develop and implement appropriate plans of action to correct deficiencies.
• Consult with and coordinate social service activities with administration, medical and nursing services, consultants, and other related departments.
• Participate in the admission of residents.
• Participate in the development and implementation of the discharge plan.
• Participate in the implementation of social care plans.
• Assist in coordinating social services programs with families, staff members, and community agencies.
• Serve as the facility’s liaison to community agencies.
• Service as the liaison to the on-site public school programs and infant development program.
• Provide information to families regarding available support agencies and financial assistance programs to meet the needs of families.
• Provide supportive counseling to families.
• Assist families in applying for services from California Children’s Services and regional centers.
• Attend Family Council meetings as a liaison to listen to concerns clarify issues and/or facilitate changes (when invited).
