Position Overview
The RN Case Manager provides care coordination, discharge planning, and utilization review throughout the acute care stay.
Responsibilities
Initial & final patient assessments
Discharge planning & post-acute referrals
Care coordination with multidisciplinary team
Prior authorizations & medical necessity review
Requirements
Active CA RN License
BLS (AHA)
2+ years Case Management experience
EPIC experience required
Knowledge of post-acute levels (SNF, LTAC, Rehab, Hospice, etc.)
