** About the Opportunity

We are seeking a **dynamic and mission-driven Program Manager** to lead and grow a portfolio of programs focused on supporting individuals, families, and communities. This role is ideal for someone passionate about improving access to services, enhancing quality of life, and building integrated solutions across health, social, and community systems.

You will play a key leadership role in managing day-to-day program operations while helping shape future initiatives that expand reach, improve outcomes, and create lasting impact.

** What You’ll Do

* Lead the **daily operations** of multiple programs focused on care coordination, support services, and community engagement
* Develop and execute **program plans, timelines, and budgets** aligned with organizational goals
* Monitor program performance using **data-driven insights** to improve outcomes and efficiency
* Collaborate with internal teams, external partners, and stakeholders to ensure seamless program delivery
* Identify risks and implement **proactive solutions** to keep initiatives on track
* Oversee intake processes and strengthen relationships with referral partners
* Contribute to the **design and launch of new programs** and services

** What We’re Looking For

* Strong **communication and collaboration skills** with the ability to work across teams
* Experience managing programs, projects, or services in **healthcare, social services, nonprofits, or related fields**
* Knowledge of relevant regulations (e.g., privacy/confidentiality standards)
* Proven ability in **budgeting, performance tracking, and project management**
* Comfortable using technology, including CRM systems, case management tools, or similar platforms
* Highly organized with strong time management and problem-solving skills
* Experience serving diverse populations or working in community-based settings is a plus
