TravelNurseSource is working with Lancesoft Inc to find a qualified Case Manager RN in Oakland, California, 94608!

Shift Details

• Shift: Day Shift
• Schedule: 5x8 Hours
• Timing: 08:00 AM – 04:30 PM
• Weekend Coverage: Every Other Weekend
• Call Requirement: Yes

Main Responsibilities

• Coordinate patient care and discharge planning throughout hospitalization
• Perform utilization review and ensure correct level of care (LOC)
• Work closely with physicians, bedside nurses, social workers, and financial counselors
• Monitor length of stay and prevent unnecessary admissions/readmissions
• Develop transition plans for post-acute care
• Conduct patient/family assessments and care coordination
• Handle concurrent reviews, continued stay reviews, and documentation in Epic
• Apply CMS, InterQual, and Milliman guidelines
• Identify barriers to discharge and arrange appropriate resources/DME/services
• Participate in interdisciplinary rounds and collaborate with the care team
• Ensure compliance with Medicare and Joint Commission requirements
• Support observation patients and appropriate status determination
• Coordinate referrals to community resources and post-acute facilities

Requirements

Mandatory

• Active California RN License
• Minimum 2 years of RN Case Management experience
• Acute Inpatient Hospital Case Management experience required
• Travel experience required
• BLS Certification (AHA preferred/in hand at submission)
• Available for lab work within 3–5 days after signing
• 7-year work history verification required

• Community Hospital experience
• Experience in ICU, ED, MS, SDU/PCU settings
