Clinical Program Manager

SUMMARY:
The Clinical Program Manager supports and promotes LACHC’s mission to show God’s love by providing quality, comprehensive healthcare to the homeless and underserved. This position provides clinical and operational oversight for the Maple Avenue Harm Reduction and Stabilization Housing (HRSH) program. The Manager ensures high-quality, coordinated care for patients engaged in Medication-Assisted Treatment (MAT), HIV, Hepatitis C, and related outreach services in both clinic and street medicine settings. The Program Manager directly supervises care coordination and support staff, including Care Coordinators, Case Managers, Certified Nursing Assistants, EMTs, Intake Coordinators, Care Team Specialists, and Peer Support Specialists. The role ensures that all activities align with LACHC’s standards of care, harm-reduction principles, and trauma-informed practice.

ESSENTIAL DUTIES, RESPONSIBILITIES, AND PRIVILEGES include:

Program Leadership and Oversight:

  • Provide leadership and daily operational oversight of services at the Maple site.
  • Oversee implementation of best practices for MAT, HIV, and Hepatitis C care, ensuring integrated and person-centered service delivery.
  • Lead departmental operations and improvement initiatives focused on patient outcomes, workflow efficiency, and quality of care.
  • Participate in department planning, data review, and quality improvement meetings.
  • Develop and maintain departmental workflows, procedures, and staff protocols that promote consistency and excellence.

Staff Supervision and Development:

  • Hire, onboard, supervise, and coach support staff at Maple Campus.
  • Manage schedules, assign duties, conduct performance appraisals, and provide mentoring to optimize individual and team performance.
  • Facilitate ongoing training and education to ensure evidence-based, comprehensive care for STIs, HIV, and opioid-use disorders.

Clinical Coordination and Patient Support:

  • Provide individual supervision and consultation to staff on complex cases, supporting problem-solving and patient-centered care planning.
  • Review assessments and care plans to ensure appropriate interventions for patients engaged in MAT, HIV, and Hepatitis C services.
  • Deliver direct patient care coordination in both clinic and field settings, ensuring timely follow-up, linkage, and continuity of care.
  • Use effective communication skills, including reflective listening, to build trust and rapport with patients facing homelessness, trauma, or substance use challenges.
  • Assess social, emotional, and economic barriers that impact health engagement and treatment adherence.
  • Promote self-management and health-literacy skills to empower patients in achieving individualized health goals.
  • Provide health education on harm reduction, nutrition, medication adherence, and preventive care.
  • Collaborate with clinical teams to ensure holistic, patient-centered care planning and problem resolution.

Bilingual Spanish preferred.

PROFESSIONAL QUALIFICATIONS:

Current, active Registered Nurse (RN) license in good standing. Bachelor’s in Nursing or a related field. Minimum 3 years of clinical experience with previous management or leadership experience preferred. History of working with individuals experiencing homelessness, mental health conditions, or substance use disorder preferred.

While performing the duties of this job, the employee will be exposed to carriers of contagious diseases such as TB, AIDS and other sexually transmitted diseases. The employee may have to deal with irate and irrational clients who may or may not be under the influence of mind-altering substances such as cocaine, marijuana, alcohol, etc.
