Job Summary

The Personal Service Coordinator provides intensive in home and community-based support, intervention, case management, and intensive care coordination services to patients and families enrolled in the Project HEALTH FSP program under the direction of a lead personal service coordinator. Actively works to improve the mental health outcomes of patients. Duties include, but are not limited to, provision of resources, care coordination, case management, and in-home and community-based support and intervention to children and families enrolled in the program.

Pay Range

Minimum $26.73 Midpoint $35.41 Maximum $44.10

Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements.

Experience

  • Required: Minimum one (1) year of community and/or mental health experience.

Education

  • Required: Bachelor's degree in health, human services, or related

Training

  • Required: None required.
  • Preferred: Training in behavioral interventions.

Specialized Skills

  • Required: Strong verbal and written communication skills; interpersonal relationship and collaboration skills. Excellent analytical and organization skills; excellent critical thinking, judgment, and problem-solving skills. Ability to speak, read, write, and understand English effectively at a level appropriate for the safe and effective performance of the job. Ability to work independently; manage multiple assignments; work well under pressure; organize and prioritize workload; recall information with accuracy; pay close attention to detail.
  • Preferred: Bilingual - English/ Spanish.

Light - Moderate energy level Lift and carry 25-35 lbs. Push/pull 50-100 lbs. (ie. empty bed, stretcher).
