Physical Therapist Job Details
- Location: Santa Rosa, CA 95401 (Sonoma County – Northern California)
- Healthcare Setting: Home Health – Geriatric Case Load
- Employment Type: Travel Physical Therapist Contract
- Assignment Duration: 13 weeks (Start: 09/29/2025 – End: 12/28/2025)
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Hours per Week: 40
- Shift: 8-hour days
- Patient Ratio: 1:1 (Up to 6 patient visits per day; 30 productivity points standard)
- Travel Radius: Occasional maximum of 50+ miles (clients grouped to minimize drive time)
- Estimated Weekly Salary Range: $2,450 - $2,510
- Active California Physical Therapist (PT) license required
- Minimum 1 year of recent skilled home health experience (within the last 3 years)
- OASIS documentation experience required
- BLS certification required
- Reliable transportation for travel within Santa Rosa and Sonoma County
- Deliver high-quality physical therapy services to geriatric patients in home health settings
- Conduct comprehensive patient assessments and develop individualized care plans
- Utilize OASIS for start of care and ongoing clinical documentation
- Provide specialized care in women's health, vestibular therapy, pelvic rehab, and case management
- Collaborate with interdisciplinary healthcare team members to optimize patient outcomes
- Maintain accurate and timely documentation of patient visits and progress
- Travel within assigned territory in Santa Rosa, with clients grouped to minimize windshield time
