Physical Therapist Case Manager: Home Health - Remote
Collaborate with PTAs to ensure effective implementation of care plans.
Help patients improve mobility, movement, and independence.
Communicate and coordinate with physicians to ensure quality care.
Evaluate and report on patients' progress.

What You'll Need to Succeed: Current unrestricted CA Physical Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Understanding of individual development and human behavior related to illness/injury, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Home healthcare experience (highly preferred).
Experience with high-acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case management fundamentals (highly desired).
Self-directed, empathetic, and a critical thinker.
Outstanding interpersonal and leadership skills.
Organized and detail-oriented.
