Lead Case Manager II
- Serve as an experienced clinical resource for case managers, providing informal guidance, case consultation, and support with complex situations
- Assist with onboarding and orienting new team members by modeling best practices and supporting workflow understanding
- Promote consistent adherence to trauma-informed care, harm reduction, and program values through daily teamwork and communication
- Facilitate or support team huddles, case reviews, or interdisciplinary coordination as assigned
- Support team cohesion, communication, and operational flow through proactive problem-solving and collaborative engagement
- Provide leadership in clinical decision-making during crises or urgent client needs, while deferring supervisory actions to the Program Manager
- Communicate consistently with internal multidisciplinary teams and external partners to support continuity of care
- Provide comprehensive case management services to individuals who have serious behavioral health conditions, including substance use disorder and are experiencing homelessness, utilizing trauma-informed and harm reduction principles and practices
- Conduct program intakes, comprehensive bio-psychosocial needs assessment, and service evaluations
- Develop, implement, and regularly update collaborative care management and housing support plans that support individuals’ needs in the areas of physical health, mental health, substance use disorders, community-based services support, social supports, and social determinants of health
- Engage in frequent client contact through scheduled or unscheduled visits, within program outreach, and telephonic support—at a minimum once per week
- Advocate for clients to reduce barriers to care and ensure access to benefits and community resources, including accessing additional benefits and related documentation
- Provide crisis intervention, safety planning, and de-escalation support
- Coordinate care across hospitals, managed care plans, behavioral health teams, housing providers, and other community partners
- Plan for discharge to appropriate housing by providing housing navigation support, coordinating referrals, housing support linkages, and housing application support
- Support clients with life skills development, scheduling, and transportation coordination
- Monitor and document client progress, including changes in clinical status and service needs
- Maintain accurate, timely, confidential documentation in compliance with agency and regulatory standards
- Support quality assurance efforts by reviewing charts, documentation, and workflow practices, and communicating concerns or improvement opportunities to program leadership
- Assist leadership in identifying patterns that may affect team performance, documentation accuracy, client care consistency, or adherence to program standards
- Participate in workflow improvement efforts, and quality initiatives
- Contribute to staff development by modeling best practices, offering peer guidance, and reinforcing trauma-informed and harm reduction approaches
- Assist leadership in performance evaluation processes by providing observations, documentation insights, and feedback to the Program Manager regarding team-wide clinical quality, workflow needs, and service delivery trends
- Maintain a flexible schedule according to client and program needs in communication with program leadership
- Participate in meetings, training, and agency-wide initiatives as assigned
- Perform additional duties as assigned
- Master’s degree in social work (MSW), Marriage and Family Therapy (MFT), or related behavioral health field, required.
- Registered with the California BBS as an ASW, AMFT, or equivalent; licensure, preferred.
- Two (2) or more years of progressively responsible experience in clinical case management, behavioral health, homeless services, or medical respite, required.
- Bilingual (Spanish/English), preferred
- Ability to manage high-acuity cases and respond calmly and professionally to crisis or emergency situations
- Ability to prioritize, multitask, and meet multiple deadlines in fast-paced, high-pressure environments
- Strong organizational skills with excellent attention to detail and accuracy
- Ability to work independently while exercising sound judgment
- Working knowledge of mental health, co-occurring disorders, substance use treatment models, and crisis intervention
- Knowledge of adult development and community and social service resources
- Excellent written and verbal communication skills
- Ability to work effectively within multidisciplinary and collaborative teams
- Ability to communicate professionally with a diverse workforce and maintain positive working relationships across all organizational levels
- Demonstrates teamwork, cooperation, and a strong sense of urgency
- Ability to establish and maintain appropriate professional boundaries with clients
- Commitment to maintaining the confidentiality of client and employee information
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Primarily office-based work environment
- Frequent use of computers, phones, and standard office equipment
- Regular interaction with staff, clients, and external partners in a professional setting
- Ability to remain in a seated position for extended periods, with periodic standing and walking
- Ability to perform tasks requiring manual dexterity, including keyboarding, filing, and handling office materials
- Ability to occasionally move, lift, carry, push, or pull materials weighing up to 25–30 pounds
- Ability to bend, stoop, reach, and grasp as necessary to perform essential job functions
- Ability to communicate effectively and exchange information verbally and in writing
- Ability to perform essential job functions with or without reasonable accommodation
