LVN / RN Home Care Case Manager

What You’ll Be Doing:

Case Management & Coordination:

You’ll help create personalized care plans for elderly clients, making sure their health needs are met while respecting their preferences. This involves regular assessments, planning, and follow-ups to ensure the care plan stays on track.

Teamwork:

You’ll collaborate with doctors, specialists, and other care providers, making sure everyone’s on the same page when it comes to the client’s health. It’s all about keeping things seamless and coordinated.

Patient Advocacy:

You’ll be the voice for your patients—ensuring they’re getting the services they need and that their concerns are addressed. You’ll advocate for their health, helping to navigate the often-complicated healthcare system.

Care Monitoring & Adjustments:

You’ll keep a close eye on your patients’ progress, making any necessary adjustments to their care plans based on feedback or changes in their health.

Documentation & Reporting:

Keeping accurate and up-to-date records of all interactions, assessments, and care interventions is key to ensuring the care we provide is consistent and of the highest quality.

Insurance & Billing Coordination:

You’ll work with insurance providers and care management organizations to make sure services are reimbursed and all paperwork is in order.

Ongoing Learning:

You’ll stay up to date on the latest trends and best practices in senior care, constantly looking for ways to improve how we care for our clients.

What We’re Looking For:

Education & Experience:

You’ll need a Bachelor’s degree in a related field (like social work, nursing, or psychology). An LVN or higher nursing qualification is preferred. Previous experience in case management or elderly care is a big plus.

Passion for Senior Care:

Understanding the unique medical needs of elderly patients and having a genuine desire to improve their quality of life is essential. You’re not just managing care—you’re making a real difference in their world.

Communication Skills:

You should feel comfortable talking with families, healthcare teams, and insurance providers. Whether it’s on the phone, in person, or in writing, we need someone who can communicate clearly and effectively.

Organizational Skills:

Juggling different patients, appointments, and documents requires someone who can stay organized and handle multiple tasks with attention to detail.

Medical Knowledge:

You’ll need to be familiar with medical terminology and have a basic understanding of how to interpret diagnostic evaluations, as you’ll be working with healthcare teams to coordinate care.

Tech-Savvy:

You don’t need to be a tech expert, but you should be comfortable using basic software for documentation and communication, plus navigating insurance and billing systems.
