On An Average Day You Will:
(Includes, but not limited to)
- Receive case referrals and review available patient information related to the case, including home visits, to determine hospice/home care needs
- Assign appropriate hospice personnel to referral case as needed
- Confer with Medical Director and Attending Physician regarding any questions as to an individualâ?s eligibility for services
- Review and evaluate each case through a variety of means such as home visits, conferences, records review, and the services provided by clinicians
- Review patientâ?s medical diagnosis, prognosis, medications, procedures and clinical course
- Discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home care services
- Available, during operating hours, to assist clinicians as appropriate
- Conduct clinical record reviews and communicate findings and recommendations to the Clinical Director and hospice personnel, as defined by the QAPI Program
- Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals
- Assist in the orientation of new employees, as needed
- Assist the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs
- Assist in the formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives â?? both short- and long-range
- Comply with accepted professional standards and principles
- Assess the health needs of the community and assist in the development of plan(s)/program(s) to meet needs
- Participate in public relation and community activities that promote the organizationâ?s role as an effective member of the hospice and health care delivery system
- Promote customer service orientation to all hospice personnel
- Participate actively in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the QAPI Program
- Perform other duties and activities as delegated by the Director of Patient Care Service
- Additional duties as assigned
Qualifications:
- A Registered Nurse license in good standing to practice professional nursing in the state
- Must have previous experience in a hospice home care setting with two years of management or supervisory experience
- An enthusiasm for providing the best possible care to patients and their families
- Understanding of the hospice philosophy and issues of death and dying
- Excellent observation, verbal, and written communication skills
- A valid driverâ?s license
- Current CPR certification
