Occupational Therapist: Home Health - Remote

What You'll Do:

Provide skilled care: Evaluate and address the needs of individuals in their homes, helping them live safely and productively.
Empower patients: Educate patients and their families on home safety, medication management, pain management, falls prevention, and chronic disease management.
Collaborate with the team: Partner with the Case Manager and other interdisciplinary team members to provide comprehensive care.
Advocate for patients: Serve as a patient advocate, ensuring their needs are met and their voices are heard.
What You'll Bring:

Current and unrestricted CA Occupational Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Understanding of individual development, human behavior, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines is preferred.
Home healthcare experience is highly preferred.
Experience with high-acuity patients is a plus.
Experience working with an interdisciplinary team is highly desired.
Case management fundamentals are highly desired.
Self-directed, empathetic, and critical thinking skills.
Outstanding interpersonal and leadership skills.
Strong organizational skills.
