Nurse Practitioner or Physician Assistant (San Francisco)

Schedule: 40 hours per week

Employment Type: Contract-to-Hire (with potential for permanent position)

Salary: Ranges from $195,000 to $210,000 per year

Experience: 2 years post-graduate experience as an NP or PA

Position Overview:

As a Nurse Practitioner (NP) in San Francisco, you will be an essential part of a leading non-profit organization committed to serving low-income people of color in Bayview and throughout San Francisco. The non-profit focuses on building empowered, clean, safe, and healthy communities, addressing unemployment, homelessness, and advocating for basic human rights.

Key Responsibilities:

• Provide direct patient care, including comprehensive assessments, treatment planning, and ongoing management of chronic and acute health conditions.
• Collaborate with an interdisciplinary team to ensure comprehensive care for patients.
• Adhere to clinical guidelines and protocols established by the organization.
• Document patient encounters thoroughly and accurately in electronic health records (EHR).
• Participate in regular team meetings and clinical case reviews.
• Provide education and counseling to patients and their families on health promotion and disease prevention.
• Coordinate referrals to specialty care and community resources as needed.
• Support quality improvement initiatives and participate in data collection and reporting activities.

Qualifications:

• Completion of an accredited Nurse Practitioner or Physician Assistant program.
• Current licensure as an NP or PA in the state of California.
• At least 2 years of post-graduate experience as an NP or PA.
• BLS and ACLS certifications (preferred).
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
