Nurse Practitioner or Physician Assistant (San Francisco)
- Compensation: $195,000 to $210,000 per year
- Required Qualifications:
- Provide direct patient care, including comprehensive assessments, treatment planning, and ongoing management of chronic and acute health conditions.
- Collaborate with an interdisciplinary team to ensure comprehensive care for patients.
- Adhere to clinical guidelines and protocols established by the organization.
- Document patient encounters thoroughly and accurately in electronic health records (EHR).
- Participate in regular team meetings and clinical case reviews.
- Provide education and counseling to patients and their families on health promotion and disease prevention.
- Coordinate referrals to specialty care and community resources as needed.
- Support quality improvement initiatives and participate in data collection and reporting activities.
- Completion of an accredited Nurse Practitioner or Physician Assistant program.
- Current licensure as an NP or PA in the state of California.
- At least 2 years of post-graduate experience as an NP or PA.
- BLS and ACLS certifications (preferred).
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
Schedule: 40 hours per week
Employment Type: Contract-to-Hire (with potential for permanent position)
Salary: Ranges from $195,000 to $210,000 per year
