Responsibilities
- Provide routine nursing care to assigned patients under the supervision of an RN or MD
- Perform clinical procedures within scope (e.g., injections, phlebotomy, wound care, sterile dressings)
- Administer prescribed medications and monitor patient responses
- Assist in developing and implementing an interdisciplinary care plan and discharge planning
- Educate patients and families, supporting shared decision-making and self-care after discharge
- Maintain organized, clean treatment areas and restock supplies as needed
- Participate in on-call duties as assigned by agency leadership

Requirements
- Active LVN/LPN license in the state of practice (California)
- Basic Life Support (BLS) Certification for Health Care Providers
- EPIC EMR experience required
- Hospice experience required; approximately two years of recent nursing experience preferred
- Education: High School Diploma or GED required; Bachelor’s degree preferred
- Valid Driver’s License preferred and candidate must be at least 21 years old if a driver's role is required
