Primary Care Nurse / Registered Nurse to cover Fresno area

As a Home Health Registered Nurse, you'll provide in-home skilled nursing care to patients by developing personalized care plans, administering medications, and supporting recovery and independence. What You'll Do

* Provide skilled nursing care in accordance with physician orders and organizational policies

* Perform comprehensive assessments and develop individualized care plans

* Collaborate with physicians, therapists, social workers, and home health aides

* Supervise and support LPNs and Home Health Aides

* Ensure compliance with regulatory standards and contribute to quality improvement initiatives

* Promote patient satisfaction and positive health outcomes

What You Bring

* Current RN license (BSN preferred)

* Minimum 1 year of nursing experience (required in California)

* Home healthcare experience preferred

* Valid driver's license and reliable transportation

* Strong computer skills and a commitment to ethical, mission-driven care

Physical & Mental Requirements

* Frequently lift, stand, and walk (35% of time)

* Occasionally bend, stoop, twist, and assist with patient positioning (10%)
