Social Worker Supervisor
The Position: Under the supervision of the Center Director, the Social Worker Supervisor is a mental health professional, helping frail and elderly PACE participants find solutions to a wide range of social problems, serves as an integral member of an Interdisciplinary Team (IDT) and responsible for performing psychosocial assessments, developing and implementing plans of care, and facilitating communication between the participant, family and CEI staff. This position serves as location contact for the discipline and supervises the location Social Workers and Social Work Assistants.
The rate range for the Social Worker Supervisor position at Center For Elders Independence is $92,150 to $138,225 annually. Placement within the range is determined based on job-related factors such as relevant experience, skills, abilities, internal equity, and market data, consistent with CEI’s annual compensation review and established compensation practices.
Duties and Responsibilities:
- Serves as the point person for the social work team at the assigned center.
- Participates in the interview and hiring process with other leaders as directed.
- In conjunction with the Social Work Functional Manager, provides orientation and training to onboard incoming social workers and social work assistants at assigned center.
- Collaborates with Social Work Functional Manager across centers to ensure standardization of best practices or protocol in the department
- Represents the Social Work department in Quality and Process Improvement initiatives and meetings.
- Reviews and approves direct report time cards for accuracy and completeness.
- Ensures there is adequate coverage in the social work team during vacations and time-off requests. Responsible for reviewing, approving, or denying requests for time off based on business needs.
- Ensures that the social work team remains current with their assessments as required.
- Completes performance evaluations and participates in employee development and performance coaching.
- Organizes Psychosocial rounds in coordination with Center team, Psychiatrists, and/or CEI LCSW team.
- Supports the standardization of best practices, supervision, and implementation of Behavioral Health training models, and internal participant referrals.
- In conjunction with the Social Work functional manager, coordinates, manages and evaluates student/intern placement with corresponding schools, universities as needed.
- Refers participants with severe mental challenges to Behavioral Health resources.
- Serves as an interdisciplinary team (IDT) member responsible for developing and implementing a comprehensive, integrated plan of care, monitors progress toward objectives by evaluating behavioral adjustments and communicates participant changes to team members.
- Completes initial assessments, re-assessments, and care plan updates for participants while continually evaluating the participants' social service needs and their families.
- Makes referrals to community resources, including those necessary for financial management, benefits eligibility, and to address related participant needs.
- Manages assigned caseload; prepares narrative and other reports, documents, and correspondence regarding participant status; documents case files.
- Responds to, investigates, and reports complaints of elder abuse.
- Conducts as-needed family and/or caregiver conferences.
- Develops and maintains working relationships with other agencies, such as the Department of Social Services, psychiatric facilities, hospitals, social agencies, and may participate in hospital and/or SNF discharge planning.
- Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families in conformance with HIPPAA principles.
- Makes home visits, hospitals, and travels to other places to meet with participants and service providers.
- Performs related duties as assigned.
QUALIFICATIONS:
- Masters’ degree in Social Work from an accredited school of social work
- Experience working with medically frail elderly desirable.
- 1-2 years Supervisory experience required with other related experience
- Demonstrated ability to work in a multicultural and multidisciplinary team environment.
- Ability to work with minimal supervision.
- Excellent oral and verbal communication and meeting facilitation skills.
- Experience working with severe mental illness is desirable.
- Bilingual ability preferred (Spanish, Korean, Cantonese, and/or Mandarin).
- Valid California driver's license, acceptable driving record and insurance with access to reliable transportation.
- Current CPR and First Aid

Unlike other healthcare plans, CEI is not a "fee-for-service" plan. It is a “capitation” healthcare plan.
