Key Responsibilities

• Provide skilled physical therapy services to geriatric patients in a home health setting
• Perform and document Start of Care assessments using OASIS
• Manage a daily caseload of up to 6 patients while meeting productivity standards
• Travel independently to patient homes within the assigned service area
• Collaborate with the care team to develop and implement individualized treatment plans

Required Qualifications

• Active California Physical Therapist license
• Minimum of 1 year of recent skilled home health experience (within the last 3 years)
• Proficiency with OASIS documentation, including Start of Care assessments
• Experience with HCHB (HomeCareBiz) electronic medical records system
• Current Basic Cardiac Life Support (BCLS) certification
• Must possess reliable personal transportation

Preferred Experience / Skills

• Experience working with a geriatric patient population in a home health environment
• Familiarity with efficient caseload and route management to minimize drive time
