Healthcare Staffing Professionals has an immediate need for a Registered Nurse to work in a medical facility located in El Monte.The Public Health Nurse (PHN) is one of three members of the Medical Consultation Team working with the Senior Clinical Social Workers in support of lead care management. DPH/CMS ECM requires a skilled, professional nurse to integrate nursing knowledge and protocols into ECM practice, who can set guidelines, and who will assist in establishing roles and expectations for the lead care managers as they facilitate coordinated care from multiple healthcare systems for ECM clients. Nursing care coordination is necessary for this child/youth Population Of Focus (POF) due to their involvement in multiple health care systems. By definition, this POF must engage medical care from other than their PCPs due to their conditions (CCS-eligible condition) or status (foster care).
PROGRAM OVERVIEW: Enhanced Care Management (ECM) is a Medi-Cal managed care benefit that addresses clinical and non- clinical needs of high-need individuals through the coordination of services and comprehensive care management. Childrens Medical Services (CMS) administers the California Children Services, the Medical Therapy Program, the Child Welfare Public Health Nursing Program, and the CMS Edelman Childrens Court Pediatric Program. ECM-CMS program will serve this particularly vulnerable ECM population of focus (POFs): Children/Youth Enrolled in California Childrens Services (CCS) with Additional Needs Beyond the CCS Condition. DPHs ECM-CMS team has extensive experience and expertise in working with this population for intensive care management and care coordination. It is anticipated that some CCS clients are also in the Child Welfare System. These clients, when encountered will be included in the initial launch. After the initial successful program launch, ECM-CMS will consider growth to fully serve both POFs. Intensive care management will be provided based on comprehensive care plans that aim at addressing the need for overall care management for the multiple systems with which the child/youth and family interact. The positions will start immediately upon acceptance and employment clearance and are initially funded through 8/31/26, with the good chance of extension.

Job Duties The PHN is the principal nursing consultant on the ECM care management leadership team, assesses lead care manager caseloads and workforce capacity to meet the need, and recommends scaled up staffing as ECM achieves sustainability.
The PHN partners with the SCSWs to ensure that the work of the lead care managers is fully aligned with the goals and principals of ECM in this medically complex POF.
The PHN will partner with the MCPs, and DPH/CMS LCM leadership to establish ECM medical care coordination performance metrics and define how they contribute to overall LCM strategies.
The PHN will provide necessary expertise as ECM partners with the MCPs to devise and measure patient health outcomes in client ECM experience, quality of life stabilization, pediatric benchmarks, medical care utilization, equity and inclusivity, and satisfaction with care management.
The duties of the PHN include but are not limited to:
Provide the lead care managers with a full understanding of the roles of various providers and types of healthcare coverage for the clients in the ECM POF;
Assign caseloads to the SCSW teams and prioritize nursing care coordination responsibilities;
Provide trainings for the LVNs and for LCM staff on medical issues;
Consult with staff on complicated medical care coordination challenges;
Define nursing care guidelines for ECM and ensure that professional standards are met;
Monitor and measure ECM client health outcomes;
Oversee medical record and health data collection, and spot-checks record-keeping in the EHR for consistency and completeness; and
Coach and mentor the lead care managers as they learn how to navigate multiple systems of care.
