Description
As an Outpatient RN Care Manager, you will manage chronic diseases and monitor medical conditions in collaboration with the interdisciplinary team and primary care providers. You will provide caregiver education and support within a licensed adult day health program, focusing on reducing hospitalizations and improving patient outcomes. Highlights- Focus on caregiver education and community health.
- Collaborate with a multidisciplinary team.
- Manage chronic diseases and monitor medical conditions.
- 1 year of prior RN experience REQUIRED (Clinic, Primary Care, Pre/Post, Home Health settings(s) preferred)
- EMR System experience preferred.
- Credentialing requirements: Current RN licensure, BLS certification, and other relevant certifications.
