Case Manager

San Diego Youth Services seeks a Case Manager in the TAY Housing Program to be based in San Diego. This is a full-time & non-exempt position reporting to the TAY Housing Program Manager.

We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing to more than 28,000 youth every year at 100+ community and school locations. Through prevention, early intervention and treatment, we also help youth before they need higher levels of care or become homeless.

  1. Meet the basic needs of youth. Hunger, poverty and youth homelessness are on the rise for many youth and their families today. We help meet these basic needs for youth. What could be more vital than food and shelter? We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing.
  2. Create positive connection. We help youth focus on positive life changes and healthy relationships as part of providing needed resources and services. San Diego Youth Services has long provided foster homes and adoption support services for families considering adopting a youth in the foster care system.
  3. Help youth develop life skills. In addition to a range of safe and stable housing options, we offer support to help youth develop the independent living skills necessary to become self-sufficient and keep off the streets.
  4. Promote overall health and well-being. Early intervention and care for the whole child are critical. We provide a broad continuum of services to help youth overcome trauma and challenges that put them at risk, including case management, counseling, mental health treatment and educational support and resources .

DEPARTMENT DESCRIPTION OR PROGRAM DESCRIPTION HIRING FOR

We provide shelter and transitional housing for children and young adults, without a home, who otherwise would have nowhere to go.

CONTEXT OF THE POSITION

TAY Housing offers transitional housing and supportive services to transition aged youth (TAY) who are homeless, at risk of being homeless within 14 days, are in extended foster care system or former foster youth in after care. Provide trauma informed case management, independent living skills development, mental health support, co-occurring services, crisis intervention, permanency planning, employment services and education services to current foster and homeless TAY population as they are transitioning into an independent living situation. Assist youth in developing skills, resource linkage, obtaining employment, pursuing education, financial planning and developing life plans that will support independence.

Essential functions include:

  • Work collaboratively and cooperatively with staff and supportive service providers within the program, the agency, and the community.
  • Attend and participate in required meetings and trainings for the Transitional Housing Program.
  • Participate in all required programs, agency and contract trainings, meetings, and consultation/supervisions.
  • Maintain accurate and timely uniform record documentation. Maintain data collection and contract compliance of records as required in multiple databases. Ability to learn County ETO system and adhere to the deadlines for all required documentation.
  • Maintain clear, effective and open communication with all clients, volunteers, staff, supportive service providers and supervisors. Maintain positive attitude within a team environment.
  • Provide advocacy, information, referral, case management, independent living skills development, mental health support, co-occurring services, crisis intervention, employment readiness, education services, financial planning, permanency planning and other trauma-informed direct services to current/former homeless and foster youth.
  • Provide outreach to prospective participants and interview youth for participation in program.
  • Complete intake and discharge processes in accordance with agency and funding requirements.
  • Maintain linkages with community referral and employment resources.
  • Provide supportive services to parenting youth.
  • Maintain collaboration with County ILS workers, ILS contractors, social workers, probation officers, attorneys or any other professionals, in development and delivery of service plans and coordination of client care.
  • Provide crisis intervention to youth living in program and assist youth compliance with service plan, using a trauma-informed approach.
  • Provide transportation when needed. Be willing to drive agency van to move youth in and out of apartments, be willing to physically move furniture, mattresses or other items on a regular basis

Perform other Duties as assigned.

REQUIREMENTS AND QUALIFICATIONS

  • Bachelor’s degree in social work or related field preferred.
  • Two years’ experience working with TAY Population.
  • Experience in crisis intervention, case management, youth participation and youth development.
  • Possess knowledge of or complete training in trauma-informed care and dual diagnosis treatment, cultural inclusivity, positive youth development, risk and protective factors.
  • Experience providing comprehensive trauma informed case management, crisis intervention, and other targeted direct services to homeless and/or foster youth would be valued.
  • Record keeping, data collection and contract compliance abilities.
  • Must possess excellent oral and written skills and be able to work well autonomously and with a multi-disciplinary team.
  • Must be proficient in Microsoft Outlook, Word, Excel and PowerPoint.
  • Demonstrate ability to work well under pressure, meet multiple and competing deadlines.
  • Well organized, detail oriented, and ability to work with minimum supervision and frequent interruptions, while always maintaining a professional demeanor. Can analyze, organize, and prioritize tasks.
  • Cooperate and collaborate with colleagues and supervisors and positively contribute to the culture of the program and agency.
  • Must be able to work with and represent a culturally and ethnically diverse community.
  • Must maintain CPR/First-Aid Certificate.
  • Must have good driving record and be willing to use their own, properly insured, reliable vehicle for the job (mileage reimbursed).
  • Must be willing and able to drive an agency van (15 passenger) to move youth in and out of apartments.
  • Employment subject to clearances, including Community Care Licensing requirements; background fingerprinting checks, driving record, medical clearances, tuberculosis testing, and drug testing.

PHYSICAL REQUIREMENTS

  • Must have ability to bend at the waist, kneel or crouch to assist students.
  • Must have the ability to hear and speak to exchange information and make presentations.
  • Must be able to lift and carry 20 pounds-lifting books, materials and equipment to execute lessons and presentations.
  • Sitting or standing for extended periods of time.
  • Walking extended lengths to move around campuses.
  • Potential exposure to physical injury from aggressive behavior.

  • Maintain confidentiality and privacy standards of service partner protected health information and other applicable information and material in accordance to agency and center policies, and applicable regulations and laws including HIPAA.

  • Employment is subject to required clearances which include tuberculosis test, fingerprinting, background checks, and drug testing.

Salary Range : $22.00 – 24.00/hour
