Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Assess patients, develop and implement appropriate physical therapy plans of care, and monitor progress in a home health-hospice context.
- Document patient findings, progress, and adjustments; communicate outcomes to the interdisciplinary team to coordinate care.
- Educate patients and caregivers on therapeutic activities, safety, and home exercise programs to support continued recovery.

- Ability to work in a home health/hospice setting
- Willingness to work day shifts (8 hours) for 40 hours per week
- Capacity to participate in a travel assignment over the contract period
- Strong communication and collaboration skills for interdisciplinary care
