Role Summary

We're seeking a compassionate, detail‑oriented Physical Therapist (Home Health) to provide in‑home therapy services to adult and geriatric patients under a physician's written orders. The PT will assess, plan, implement, and evaluate individualized plans of care while educating patients, families, and caregivers to promote safety and independence at home.

Key Responsibilities

• Provide physical therapy services to patients according to a physician's written orders .
• Assess and evaluate therapeutic/rehabilitative/functional status.
• Develop a plan of care and treatment ensuring alignment with the comprehensive assessment.
• Participate in the development of the patient's total plan of treatment with the interdisciplinary team.
• Initiate the physical therapy program and instruct other personnel and/or family members and caregivers in specific phases of therapy with which they may assist in patient care.
• Prepare and submit clinical and progress summaries based on identified and attained goals; submit copies to the Director of Nursing .
• Participate in discharge planning , as appropriate, ensuring safe transitions and continuity of care.
• Document timely, accurate, and complete visit notes in the EMR; communicate changes in condition promptly.
• Adhere to agency, state, and federal regulations and professional standards of practice.

Qualifications

1. Graduated with a bachelor's or higher degree in physical therapy from an APTA-accredited physical therapy program.
2. Current and unrestricted Physical Therapist (PT) license in the State of California.
3. Two (2) years of appropriate experience as a physical therapist.
4. Current CPR certification.
5. Valid driver's license.

Preferred:

• Home Health experience.
• Reliable transportation and auto insurance; comfortable traveling within assigned territory.
• Strong assessment, documentation (EMR), and communication skills; ability to work independently in the home setting.

Typical Schedule & Coverage

• Part‑time caseload with flexible day(s)/time(s).
• Service area: Monterey County - Monterey, Carmel, Pacific Grove, Seaside, Marina, Salinas, Pebble Beach, Carmel Valley, Hollister, Watsonville, and surrounding neighborhoods.
• Mileage: reimbursed for travel between patient visits within assigned territory.
