Home Health RN Job Details

• Location: Irvine, CA 92612 – Prime Southern California location with excellent parks, shopping, and dining
• Employment Type: Travel Nurse (Locum Tenens)
• Healthcare Setting: Home Health Care
• Shift: Day shifts, Monday to Friday, 8:00 AM – 5:00 PM
• Assignment Duration: 13 weeks (Start: 09/29/2025 – End: 12/28/2025)
• Hours per Week: 40
• Estimated Weekly Salary Range: $2400 - $2460
• Patient Ratio: 1:1 (Up to 6 patient visits per day; 30 productivity points standard)
• Travel Radius: Occasional maximum of 50+ miles; routes efficiently grouped for minimal drive time

Registered Nurse Qualifications

• Active California Registered Nurse (RN) license required
• At least 1 year of recent skilled home health RN experience (within the last 3 years)
• Proficiency with OASIS documentation
• Current BLS certification (American Heart Association preferred)
• Reliable personal transportation for patient visits

Homecare RN Responsibilities

• Deliver skilled nursing care to geriatric patients in their homes throughout Irvine, CA
• Perform comprehensive patient assessments and develop individualized care plans
• Complete accurate OASIS documentation for start of care and follow-up visits
• Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
• Educate patients and families on disease management, medication administration, and home safety
• Maintain timely and accurate clinical records in compliance with home health regulations
