Pay: $2,927 per week
Shift Information: Days - 5 days x 8 hours
Contract Duration: 13 Weeks

About the Position
Shift
- Day Shift: 5x8 hours
- Hours: 08:00 AM – 05:00 PM
- Schedule: Every other weekend required
- Float: May float to other UCSF facilities (within 75+ mile radius)
Responsibilities
- Manage pediatric patients’ care plans from admission through discharge
- Coordinate care with interdisciplinary teams (physicians, therapists, social workers, etc.)
- Perform discharge planning and arrange post‑acute services
- Conduct utilization review and ensure appropriate level of care
- Work with families on education, resources, and transition of care
- Navigate complex cases in NICU/PICU and general pediatrics
- Document all case management activities in the hospital’s charting system
- Ensure safe, timely, and effective patient transitions

Licenses & Certifications
- CA RN License (Required)
- AHA BLS (Required)
- COVID‑19 Vaccine + Booster required (Medical/Religious exemptions only)
- Flu Vaccine: Unknown
Experience Requirements
- 2 years Case Management experience (Required)
- Discharge planning experience required
- Travel nursing experience: Required (within the last 12 months)
- Pediatric Case Management experience: Required
- NICU / PICU experience: Required
- Trauma Level II experience: Required
- Trauma Level I experience: Preferred
- Community Hospital experience: Preferred
- Teaching Hospital experience: Preferred
Education
- BSN required
- Master’s in Nursing or Healthcare-related field: Preferred
