Responsibilities
- Provide physical therapy assistant services under the supervision of a licensed physical therapist in an LTAC/SNF environment.
- Implement and adjust therapeutic exercise programs, gait training, range of motion exercises, strengthening, and modalities as prescribed.
- Monitor patient progress, document treatment response, and communicate changes to the supervising PT.
- Ensure patient safety during mobility and daily activities; support discharge planning and home exercise instruction.
- Maintain accurate, timely documentation in the patient record and participate in chart reviews as needed.
- Collaborate with the PT, nursing staff, and therapy colleagues to coordinate care and meet patient goals; adhere to infection control and safety policies.

Requirements
- Active California Physical Therapy Assistant license or eligibility to obtain licensure.
- Associate degree in Physical Therapy Assisting.
- Experience in LTAC or skilled nursing facility settings preferred.
- Strong communication and teamwork skills; ability to document clearly and work within a team-based model.
- Ability to work in a rotating schedule and travel as assigned.
