Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist providing home health services in Sunnyvale, CA. You will evaluate and treat patients using medically prescribed physical therapy programs, document clinical findings and skilled interventions, and collaborate with the clinical team to support patient function and discharge planning. You may supervise Physical Therapy Assistants and perform initial admission/evaluation visits consistent with professional scope of practice.

Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapy license required to start the assignment.
• Current BLS certification from the American Heart Association or American Red Cross may be required.
• Experience in home health settings preferred.
• Ability to perform initial admission/evaluation visits and document OASIS assessments within required timelines preferred.

Preferred Qualifications

• Experience supervising Physical Therapy Assistants preferred.
• Familiarity with documentation of skilled interventions, goals, and discharge planning preferred.

Assignment Details

• Shift hours: 8:00 AM - 5:30 PM (schedule can be flexible based on patient availability).
• Territory includes San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, Millbrae, Hillsborough, Burlingame, San Mateo and nearby communities.
