- 2-year acute care Hospital Case Manager experience performing Care Coordination and Discharge planning
- Travel Experience Required
- HOSPITAL EXPERIENCE IS A MUST
- Current California Registered Nurse License
- Certfications: BLS
- Charting System: Epic
